Submitting your entry requirements through Adobe Sign

You will receive Ā a series of emails coming from your program assistant through Adobe sign.

These emails will allow you to submit your entry requirements:

  • BSN Acknowledgement Forms
  • Immunization Forms
  • Certificates: SFA, BLS, AM and OT (Please attach official copies, we don’t accept temporary ones).
  • TB test

The first email will look similar to the image below.

 

If you canā€™t see the College logo, go to your email settings and ā€œtrust the senderā€.

Future emails should look like this:

 

 

Adobe will send you automatic reminders until you submit your forms or certificates.

Tips:

  • PCs and Laptops sometimes work better than tablets and phones.
  • If MAC throws an error use Windows.
  • Try different browsers.
